If you decide to fill out an application for prescription assistance using the Application Wizard, you will be asked to provide personal information, such as name, address, phone number, birth date, social security number, and other information.

Use of Information:

We store your eligibility and wizard information so that we can send you reminders about your medication programs and fill out any new medication applications and refill applications.

We may use your information to: (a) process and respond to your inquiries and for the purposes for which you provided the information, such as to receive email updates and follow-up phone calls to confirm you have applied for and received your medications; (b) improve the content and navigability of the Website; and (c) enforce our Terms of Use.

We do not sell, rent, loan, or lease any personal information collected at our site. We may share your information with the proper authorities in response to a subpoena, legal order or official request, or when we believe you have acted in violation of our Terms of Use.

We do not share the email addresses on our lists with anyone outside of RxMedsAdvisor staff and authorized agents.
